A Strategic Formation
John Ford had a legacy vision to “go deep” by reforming healthcare from the ground up. He identified East Texas to incubate a laboratory for an independent managed primary care model ” under the radar” with less competitive barriers to launch a captive regional delivery alternative. Long before the ACA or MACRA, John believed employers and taxpayers needed a meaningful cost and performance alternative to high-cost insurance offerings. Building and pivoting on a core staff model primary care group structure, Phynet, John has grown participation to include over 300 physicians in upper East Texas and Southern Arkansas region. John has integrated other “low cost-high skill” services including home health, hospice, outpatient rehabilitation, chronic care management, and 24/7 care using performance-driven data analytics to become an intelligent, efficient, and prevention-minded healthcare delivery system. In 2017, Prime Care Managers ACO was approved by the Center for Medicare and Medicaid Services (CMS) to provide total care management for 22,000 Medicare Beneficiaries in the region.

2013 – TPR developed its Care Management division, Integrating clinic and home based coordinated pre-acute activity |
2014 – PrimeCare Home Health achieved a 100 census. |
2016 – Triad combined Phynet with other idependent primary care to successfully file as PrimeCare Managers, LLC ACO (PCM) |
2017 – Triad funds the developnment of a proprietary data analytics and informatics platform |
2018 – PCM achieved its first positive savings for Medicare, a 1.8% savings rate |
2018 – PCM doubled in attributed lives with the participation of the Diagnostic Clinic, an independent multi-specialty group in Longview, TX |
2018 – Triad signed to perform Care Management servic es for the Diagnostic Clinic |
2018 – Triad acquires Longview Outpatient Physical Therapy LLC, as part of its integrated preacute service strategy. |
2019 – PCM received its first shared savings payment from Medicare, achieving a 3% savings rate |
2019 – Triad implements its proprietary “Cohort Manager” stratification and workflow management tool |
2019 – Triad implements its virutal and remote monitoring programs |
2020 – PCM grew by 50% by adding the Participation of the Collom& Carney Clinic |
